When calling the verify benefits, the following questions should be asked:
- Is a referral from the primary dentist required?
- What is the maximum?
- Is it a lifetime maximum or annual maximum?
- Have any orthodontic benefits been used?
- What % is the maximum paid at?
- What is the effective date?
- Is there a waiting period for orthodontics?
- Is there a deductible and if so, is it a one-time or annual deductible?
- Are claims paid monthly or quarterly?
- Are claims paid automatically or as bille
