When calling the verify benefits, the following questions should be asked:

  • Is a referral from the primary dentist required?
  • What is the maximum?
  • Is it a lifetime maximum or annual maximum?
  • Have any orthodontic benefits been used?
  • What % is the maximum paid at?
  • What is the effective date?
  • Is there a waiting period for orthodontics?
  • Is there a deductible and if so, is it a one-time or annual deductible?
  • Are claims paid monthly or quarterly?
  • Are claims paid automatically or as bille
